Using CK-MB and hs-cTnT to Predict 30-Day Mortality After CABG: A Combined Biomarker Risk-Stratification Approach.

JACC. Advances · 2026-09-25

Winter-Pölzl Leo, Kletzer Joseph, Lohmann Ronja et al.

This multicentre cohort included 9,856 patients undergoing isolated coronary artery bypass grafting. Thirty-day all-cause mortality was 2.0%. Each doubling of postoperative CK-MB and hs-cTnT was associated with higher mortality independently of EuroSCORE II: adjusted HRs were 1.95 (95% CI 1.79–2.12) and 1.71 (95% CI 1.61–1.82), respectively. Discrimination was similar for CK-MB and hs-cTnT (AUC 0.787 versus 0.784; P = 0.99). In the combined analysis, increased mortality risk was identified only when CK-MB was in the top tertile alongside middle- or top-tertile hs-cTnT.

Clinical bottom line

Postoperative CK-MB and hs-cTnT provided prognostic information after isolated CABG. Their combined pattern may support risk stratification, but this study does not establish that biomarker-guided surveillance improves outcomes.

Study limitation: Observational associations do not establish clinical utility, and the abstract reports no external validation of the combined biomarker approach.
